About Us

Our devotion, commitment and concern for people and their pets has helped us grow and earn the loyalty of clients and employees. We have been able to provide needed veterinary care to animals that would otherwise go without. We have truly lived up to and will continue to work toward our beliefs and the concept that "No act of kindness, no matter how small, is ever wasted."

Community Involvement:
Companion Pet Clinic Salmon Creek is dedicated to the community we serve. We are proud to honor the Humane Society for SW Washington and the Oregon Humane Society's "First exam free" coupons for newly adopted cats and dogs!

We also work closely with Furry Friends, a no-kill cat shelter in Vancouver, WA. We provide spay, neuter, and most medical services for their cats. We even started an annual holiday giving tree in their honor to help get them the supplies they desperately need. Check out their adoptable cats at http://furryfriendswa.org/

Clinic Policies:
Payment is due in full at the time services are rendered. For surgical procedures a deposit of at least 50% is due at drop off, with the remainder to be paid in full at time of pickup. We accept Care Credit, all major credit cards, and cash. We offer a 5% discount when the entire bill is paid in cash. The discount does not apply to exams, vaccines, or routine surgery.

An exam is required for the administration of a rabies vaccine per Washington State law. Additionally, we require a current yearly exam and current rabies vaccine for all procedures. Washington State requires that all cats and dogs be current on their rabies vaccine. Clark County and the City of Vancouver also require all pets to be licensed.

All pets should be current on flea prevention if hospitalized or having a surgical procedure. If live fleas are seen, we will administer flea preventative medication, which the owner must pay for upon pickup.

All medication refills require 24-48 hours notice, as well as a current yearly exam on file. Some medications may require blood work every 6 to 12 months.

For your pets’ safety, dogs must remain on leash, and cats must be contained in a cat carrier when entering/leaving the clinic, and while waiting in the lobby. If you do not have a leash we have them available to borrow. If you do not have a cat carrier we have cardboard carriers available for purchase.

Holley Crooks, DVM

Dr. Crooks grew up in Phoenix, AZ and attended Northern Arizona University in Flagstaff for her undergraduate degree. Go Jacks! She went on to veterinary school at Washington State University. Go Cougs!

She and her husband have lived in and around Vancouver since she graduated in 2007. She has been working at CPC - Salmon Creek for 8 years.

Dr. Crooks has a huge appreciation for the human-animal bond and is thrilled to have a career that lets her support and strengthen the bond in the lives of others. She also enjoys being a veterinarian because every day is different and she is never stuck behind a desk.

As a veterinarian, Dr. Crooks has accumulated a house full of critters. Her pack of four cats and three dogs are ruled by a cunning little Chihuahua named Lentil. Volunteering with Feral Cat Coalition, practicing hot yoga, and roller derby are some of her passions.
